This page explains what this website collects and what happens to it. It is written to be read, not skimmed past.

What the contact form collects

The contact form asks for your name, email, phone number, how and when you would like us to reach you, and a short message. We use it only to reply to you and to find a time for a visit.

What it does not collect

Please do not put medical details in a message. We ask for your health history in person, on a paper form. This site does not use advertising trackers, and it does not run an online scheduler.

Your records

We keep your records private and share them only as the law allows. You can ask to see them or to have a copy sent to your doctor.
